The COLLABORATIVE HEALTHCARE GROUP offices are located in Suite 102 on Floor 1 of the 20th Avenue Building. The building is on the Campus of Baptist Hospital at the corner of 20th Avenue North and State Street. The building is easily accessible from West End Avenue, Charlotte Avenue, Church Street and I-440. Enter the Tennessean parking garage off of State Street between 19th and 20th Avenue North. Free parking is available in the adjacent parking garage (in blue on map). If you park in the garage, please take the elevators to the second floor of the garage. From there you will go through double doors and proceed down a ramp to the 20th Avenue Medical Office Building. Our offices are on the left as you enter the main building. Please remember on which level of the garage you parked. |
